Official Transcripts

Students can request the official transcripts online, or through the form. Form needs to be electronically filled in. It takes approximately two weeks for the transcripts to arrive.  If the transcripts are to be sent to Kosovo: On the PLEASE MAIL part of the form, you need to write “Send to Kosovo”.

Add/Drop/Audit

If the add/drop period has ended and you are told by your advisor to fill this form in.

Course Withdrawal after Deadline

To be filled in by students withdrawing from one course after the twelfth week of a term. To be accepted, this form must have the instructor and department head signature. Department Head refers to the Manager of RIT/RIT Kosovo (A.U.K) affairs.

Minor Authorization/Change of Minor

This form is required for students graduating with a minor.

Concentration Form

This form if filled in manually by the student. This form is filled in during the sophomore year in order to declare concentrations. If a student wants to change concentrations or minor the same form is used.

Apostille Request

When a diploma (or transcript) is to be used in a foreign country, it may be necessary to have the document authenticated. An authentication certifies the signature and the capacity of the official who has executed the document, and may also authenticate the seal of the official.

The NY Secretary of State authenticates public documents for use in foreign countries. Only public documents issued in New York State, which are signed by a notary public or other public official will be authenticated. The country of destination determines whether the authentication is in the form of an “Apostille” or a “Certificate of Authentication.”

Change of Name

A student who has changed their name needs to update their information in their official student record. In order to proceed with a name change, the student must present proof of name change with either one of these: driver’s license/birth certificate/court order/passport, marriage certificate (ID cards are not accepted).

Diploma Replacement

The cost per diploma replacement is $25. Please complete all requested information below. To make a payment with your credit card, go to rit.edu/registrar/diploma-replacement . Please allow 4-6 weeks for delivery.

Institute Withdrawal

This form should be completed by the student in conjunction with his or her advisor. All requested information on the form must be complete. This form will become part of the student’s permanent record.

Leave of Absence/ Return from Leave of Absence

This form is to be filled in cases where a student needs to take a leave for a semester due to personal/other reasons or when they are returning from their leave of absence. It should be completed by the student in conjunction with his or her advisor. All requested information on the form must be complete. This form will become part of the student’s permanent record.

Repeat of Grade

Students do not have access to this form. Contact advisor for a copy.

This form is to be filled in when students re-take a course but the system does not recognize it as being the same course (could occur in cases where the course codes change). The last grade earned will stand as final and figure into the earned hours and GPA.The previous grade will remain on the student’s transcript with the following statement: “The class above has been repeated.” Classes taken at other instuitutions may not be used to remove a grade earned at RIT.

Change of Grade

This form should be used by an instructor to change a grade due to a grading error, to change a grade from an incomplete “I” grade to an earned grade, or to extend an incomplete grade. Course information can be found in the professors SIS or term schedules sent out by the academic affairs.

Enrollment Correction

This form is to be used to correct an enrollment for a previous term. If grading has occurred this form can also be utilized in the current term. A grade must be assigned for adds on this Enrollment Correction form.
